We cannot diagnose but have your medical team considered 'peripheral neuropathy' (in your case 'polyneuropathy', as multiple areas are affected)? You do seem to match the symptoms.
A virus, diabetes or the side effects of some medicines (such as cholesterol drugs) are the most common causes, so I wonder if the pain followed the start of any such changes?
If diabetes is ruled out, I'd consider looking at improving your circulation since the pain is so varied in its location.
Please consider some Ginkgo biloba, though be sure to buy one that is 'holistically standardised' from whole leaf to gain best results and in case you are on blood thinning medication.
