The menopause is not a static state, your hormones can go up, down or stay level for months at a time.
Usually when symptoms come back and you are taking something it can mean that the hormonal change is much greater at that point and the medication may not be effective enough.
Stress, illness, over exercising, change of diet can do this as well.
I would just let your doctor know that the HRT is not helping so much just now and they may be able to up the dose or change to a better one.
